I had purchased a Lee Malia RD for my wife and it did not come with the COA.

She is very sentimental ( has saved EVERY card I have given her in 37 years of marriage). She contacted Epiphone and had a new COA and a free Epiphone tshirt in a week!

Great Customer Service all around.
