Kramer Assault EVH 5150?

The Kramer Assault was made in Korea by the Kramer by Gibson Company from 1998 to 2005.

So that , "Made In USA" painted on the back is a mystery.

Also, that neck plate has the EVH logo, and the Fender "F" logo.

It's doubtful that the Kramer by Gibson Company  would ever put a Fender logo on anything they manufactured.

The neck plate looks like it could have come from an EVH Stripe Series guitar.

Those models came out in 2013, eight years after they stopped making the Kramer Assault in 2005.

The first EVH Company guitar, the Wolfgang came out in 2009.

My guess is this is something someone created themselves.
