How Can I discover the model of my Gibson with the Serie no?

Yes the LPS+  means Les Paul Standard  Plus with the plus meaning AAA flamed top. I think the other letters describe the finish (i.e vintage sunburst) but cant quite make it out. Year is the first digit and 5th digit (04 for 2004) of the serial number.

NH = nickel hardware?

Not quite.

Look like it could be VS for the color, I'm only guessing that could be Vintage Sunburst.

N is nickel hardware

H is hard tail

1 is for made for domestic market.

Contact Gibson, they can tell you for sure.

Les Paul Standard 50s (neck profile) vintage sunburst finish & nickel hardware. The "+" symbol was used on the LP Standard with transparent finishes, these all featured a figured maple top, there was not a "plus" model variation at that time in 2004.
