Conventional wisdom would have you believe that US made Gibsons/Epiphones always had two hole truss rod covers, but I've seen a few three-hole versions, especially from the late 60's, early 70's period. This was not the best period for Gibson quality or consistency, and I think sometimes the channel for the truss rod gout routed a little too close to the nut, so they couldn't screw the truss rod cover down in the wood just above it.
